If Support Coordinator funding has been included in your plan, you will find it under the Capacity Building Budget. If you require support to determine the level of funding, and amount you have received, please contact us.  Alternatively, if you do not have Support Coordinator funding in your plan and feel as though you are eligible we can support to to submit a request to the NDIS.


Currently, Twin Hearts Specialist & Support Coordination is an independent provider. We are not yet NDIS Registered - This means we can provide support if your budget is Plan or Self Managed.  Please see below for more information if your plan is Agency Managed.


If your budget is Agency Managed and you would like to access our supports please contact us and we can support you with submitting a plan variation request to the NDIS.


Yes, you have the choice and control to change your Support Coordinator. Notice periods should be outlined in your current service agreement.
